Please fill out the form for the desired sport and with all the applicable information. Upon filling out the form, please reference the Sports Programs page for current fees and then mail the form and fee to the PO Box address indicated at the top of the form.

If you are interested in becoming an Optimist Club member, please click on the Optimist International Registration Form link, download the form, fill out the appropriate information and mail it to the Fortville Optimist Club PO Box.
